POSITION DESCRIPTION:
A Clinical Pharmacist is responsible for optimizing patient care through effective medication management, direct patient counseling, and collaboration with other healthcare professionals. Key responsibilities include reviewing patient records, assessing medication plans, monitoring patient outcomes, and educating both patients and healthcare staff. The role focuses on ensuring that medication use is safe, effective, and cost-efficient.
Key Responsibilities:
- Interact directly with patients to evaluate their conditions, medical history, and preferences in order to determine the most appropriate medication regimen.
- Review prescriptions, monitor drug therapies, and design, implement, and adjust pharmacotherapeutic plans.
- Work closely with physicians, nurses, and other healthcare professionals to provide drug information, offer recommendations, and participate in multidisciplinary care rounds.
- Educate patients on proper medication use and provide training and information to healthcare staff on medication management and pharmacology.
- Maintain comprehensive patient records, including treatment plans, interventions, and progress notes.
- Assess patient progress and outcomes, monitor medication efficacy and potential toxicity, and interpret laboratory results.
Qualifications:
- A Master’s degree closely related field from a recognized institution (PhD preferred).
- At least three years of professional experience in the healthcare, pharmacy, or higher education sectors.
- Demonstrated excellence in teaching, with a strong commitment to student learning and academic integrity.
- Experience in curriculum design, course development, and interdisciplinary academic collaboration.
- Ability to integrate contemporary research and industry trends into assessment and academic planning.
- Full professional proficiency in English.
- A pedagogy or teaching certificate from a recognized institution.
- Experience with accreditation standards and university ranking processes.
